What should I do about the physical disability certificate, expenses for medical services and support for persons with disabilities, medical support for the severely disabled and other welfare services if I move?
After registering your new address, you are requested to follow the procedures for changes in status for each of the welfare services that you are using. You should contact such bureaus as the bureau in charge of welfare for the disabled at the municipality where you have registered your new address.
If you have asked social workers or others to conduct procedures on your behalf, you should give them your new address, and make sure that they conduct the required procedures.

Physical disability certificate
Procedures
  • You are requested to follow the procedures for changing your address on the certificate at the municipality of your new address.

Payment for medical services and support for persons with disabilities (medical rehabilitation services)
Procedures
  • There are two cases:
    No.1, the municipality of your new address continues the payment for medical services and support for persons with disabilities that you received from your old municipality, just by registering the change of address.
    No.2, you are requested to apply for a new registration.
    As the procedures vary depending on the circumstance of each person, please contact your municipality for details.
  • You are also requested to change registration of your designated medical institution in cases where the institution that you are receiving medical treatment from changes.

  • Personal seal
  • Health insurance card
  • Tax certificate (Please contact personnel in charge of welfare for the disabled to confirm which year's certificate is required)
  • Designated doctor's statement (in cases of new registration)
Medical support for the severely disabled (welfare medical services)
Procedures
  • A new certificate will be issued by the municipality that you move to.
  • As medical support for the severely disabled (welfare medical services) is operated by separate municipalities, income limits and content of benefits may vary. Please inquire at the bureau in charge.

  • Tax certificate (Please contact personnel in charge of welfare for the disabled to confirm which year's certificate is required)
When I move, what should I do about the specified disease medical treatment beneficiary card, the congenital blood coagulation factor deficiency disease medical treatment research beneficiary card and/or the specified chronic childhood disease medical treatment beneficiary card? (In cases where a person has hemophilia.)
As each system has different procedures, please refer to the chart shown below. If you have asked social workers or others to help with procedures on your behalf, please show the persons your new address and make sure that they conduct the required procedures.
