What is the Severe Disability Medical Expenses Assistance Program (Welfare and Medical Service)?

The Severe Disability Medical Expenses Assistance Program is for persons holding a Physical Disability Certificate with a disability rating of grades 1 – 3 (it differs depending on local governments) who meet the income requirement. Governments have individual titles for this program such as “medical assistance for the severely disabled” or “medical welfare for the disabled.” Through this program, medical fees for all diseases are either reduced or exempt. (There may be a 500-yen fee for some government jurisdictions.) However, hospital meals will be charged. Unless otherwise approved, the program can only be used within the prefecture of residency.

What happens if I see a doctor in another prefecture?

For consultations out of the prefecture, copayment for the medical fee must first be paid in full. Submit the receipt and request for reimbursement to your local public office for payment into your bank account. Please be aware that discussion may be allowed between the Federation of National Health Insurance Organizations outside of the prefecture and your local government. Seeking prior advice at your hospital is recommended.

How do I apply for the Severe Disability Medical Expenses Assistance Program?

Submit necessary documents to your local government welfare office. This is often done when picking up the Physical Disabilities Certificate.
