Severe Disability Medical Expenses Assistance Program (Welfare and Medical Service)
What happens if I see a doctor in another prefecture?
For consultations out of the prefecture, copayment for the medical fee must first be paid in full. Submit the receipt and request for reimbursement to your local public office for payment into your bank account. Please be aware that discussion may be allowed between the Federation of National Health Insurance Organizations outside of the prefecture and your local government. Seeking prior advice at your hospital is recommended.
How do I apply for the Severe Disability Medical Expenses Assistance Program?
Submit necessary documents to your local government welfare office. This is often done when picking up the Physical Disabilities Certificate.
What you need to apply for the Severe Disability Medical Expenses Assistance Program
1. Health insurance card
2. Proof of income for the previous year (two years prior if applying between January and July).
This can be obtained through the taxation division of the local public office where you resided on January 1. Be advised that you may need to go to the office in your previous jurisdiction if you have recently moved.
3. Documents to prove grade of disability (Physical Disability Certificate, Annuity Certificate, etc.)
When can I start using the severe disability medical assistance?
This assistance is effective as of the first day of the month of issuance.
What is the difference between the severe disability medical assistance and the medical payment for services and supports for persons with disabilities?
Medical assistance for severe disabilities and medical rehabilitation are summarized below.
*Please note that assistance for severe disabilities is not the same with all local governments. Please consult your local government for details. The information below is relevant to the City of Hiroshima.
As medical payment for services and supports for persons with disabilities is applied to those with a certain amount of income, most individuals rely on the medical rehabilitation program. Combining both programs alleviates copayment expenditures.
